Please check with your Insurance Company and EAP benefits The insurance and EAP companies will give you information regarding your benefits. Also, ask if an authorization number is necessary for this provider. You should ask your Insurance what your responsibility is up front and if there is a deductible that needs to be met for the year. Ask how many sessions you have for both Insurance and EAP benefit per year. When talking to the Insurance Company check to see if a formal referral is needed from your PCP and how to proceed with obtaining a referral.

If you have a company not on the list please check, as EAP and Insurance companies have multiple products and sometimes company mergers occur w/ relabeling.
If I am not on your insurance company’s panel I can provide paperwork for out of Network benefits.
Payment
Payment is expected at the time of service.
During Covid -19 Major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, or you have not notified the office at least 24 hours in advance, you will be required to pay the full cost of the session.
IL and LA Currently contracting is in place for many Insurance and EAP’s. Please check with your plans coverage.
